There is a story that the investigators found an installation defect. I do not know how true that the story is but a question comes to mind from my own experience in working in a repair depot that did military work. they used ot have a Company inspector and a military inspector to check the equipment before it was shipped out to the Radar Site. Then later on these companies talked the military into not have a company inspector to check the equipment - just have the Technician show it to the Military inpector and he in this case would talke the military inspector to aaccept the equipment. The last Radar that I was involved in - in my opinion never really worked right. Matter of fact there was a airborne fire control equipment that was red tagged out of the factory and had to be remanufactured in St Louis. The manufacturers have talked everybody into accept as is from factory that it is a waiste of money to have other inspectors checking on the equipment. Don't the States or who ever is in charge of these Bridges and roads have real inspectors to check the work. I know there is a problem with inspectors with city quality control about checking home building as they seem to just check the paper work unless the home is built by the person going to move into house. I know the house that my son bought had broken concrete in the Garage and I am not sure whether the it was ever really repaired.The states should have a quality control person on the job as the bridge or roadway is built to keep the troubles down. But where are the quality people to be found to do the job. I remember back in the 30's the salemen knew things about what they were selling that is not known by the people selling merchandise now. It seems like they all memorize words but really do not know the the equipment. Is this what is known by the quality control people. I remember the last radar installation they brought in a young quality control person and all he knew was the words from a book and the wiring in radar set went bad. He did not know things that should have been known to make it work. Is this the same type of problem in the Bridge and road building that is going on ? I am starting to ramble so better quit.
